i recently replaced my alternator... a few weeks later the fan belt came off and now refuses to stay on once the car has started... the adjuster belt on the alternator is also difficult to keep in the bracket when tightening it and i have a feeling these two problems are connected... i'm wondering if anyone knows what the problem is or if i have lost part of the adjuster bolt

I would say clearly there is something wrong with the bracket or tension hardware. I lost the locking bolt out of mine, I probably forgot to tighten down on it.
